We will try to schedule your requested appointment date/time that you request but we can not guarantee your appointment until after we confirm it with you. If there are certain days and/or times that work better for you, please include that information. If you are a new patient, please allow two hours for your initial visit as we need to conduct an initial examination. If you are having an emergency, do not use this form and call us immediately.
